Registration/Registración
If your child was admitted during the Enrollment Lottery and you have accepted the offer, we will need the following documentation in order to complete your child's registration:
Registration Form & Emergency Card
Home Language Survey
Physical Form & Immunization Record
-Your child's doctor needs to complete this form. Please note that if your child has been to the doctor in the last 12 months, it will not be necessary to schedule an appointment to have the form completed.

Dental Form
-Your child's dentist needs to complete this form.
-You can email this form to Ms Karina.

Parent/Legal Guardian ID*
Proof of address*
*You should have already provided this when you completed the application for the Enrollment Lottery. Office staff will inform you if we are still in need of this.

Si su hijo(a) fue admitido durante el Sorteo de inscripción y usted ha aceptado la oferta, necesitaremos la siguiente documentación para completar la registración de su hijo(a):

-El médico de su hijo debe completar este formulario. Tenga en cuenta que si su hijo(a) ha ido al médico en los últimos 12 meses, no será necesario programar cita para que completen el formulario.

Identificación del padre/tutor legal*
Comprobante de domicilio*
*Debería haberlo proporcionado cuando completó la solicitud para el Sorteo de inscripción. El personal de la oficina le informará si aún lo necesitamos.
